Livonia, MI – Trinity Health (“Trinity”) is providing notice of a data incident that may have affected a limited number of individuals’ information. Trinity hospitals and providers participate in automated electronic data exchanges called Health Information Exchanges (“HIEs”) to support patient care, quality, and efficient care coordination. Federal regulations require that health information be shared electronically between doctors and hospitals involved in the treatment and care of shared patients. On January 13, 2026, Trinity was notified that certain companies which had obtained specific patients’ records via an HIE may not have obtained those records for the permissible purpose they provided. The HIE has suspended access to patient records by these companies while it investigates this incident. The type of information identified varied by individual but may have included name and one or more of the following: clinical care details, demographic information, insurance information, and potentially driver’s license number.

As a precautionary measure Trinity is providing complimentary credit monitoring to potentially affected individuals. To obtain more information about this incident or enroll in these services, individuals should contact the dedicated assistance line at 1-833-877-5364, Monday through Friday, from between 7:00 AM and 7:00 PM CT, excluding holidays. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors. Individuals may also place a fraud alert or credit freeze by contacting the credit reporting agencies: TransUnion 1-800-680-7289; Experian 1-888-397-3742; Equifax 1-888-298-0045.
